Description
In just three weeks, October 2, 2018, the PMMC Health Institute begins hosting Take Charge of Your Health, a Chronic Disease Self-Management Program (CDSMP). The program is a series of workshops, designed for adults age 55 and older* who want to take charge of their ongoing health problems such as arthritis, MS, fibromyalgia, COPD, diabetes, high blood pressure and heart disease. Developed by Stanford University, the program has been proven to help adults with chronic diseases find practical ways to deal with pain & fatigue, discover better nutrition & exercise choices, and learn better ways to talk with their family and doctor about their health. (It has actually been shown that program participants have fewer visits to physician offices and emergency rooms, even two years after participants completed the workshop!)
This FREE six-week program will be held every Tuesday from October 2 through November 6, 2018 from 9:30 am-12:00 noon at the PMMC Health Institute, 1975 Melissa Lane, Aurora 60506. Living a Healthy Life with Chronic Conditions, an invaluable resource book for anyone living with a chronic illness, is also provided free. Registrants need to call 877.737.4636. The workshop is offered as a partnership between Presence Mercy Medical Center, Aurora and Presence St. Joseph Hospital, Elgin.
